Check out the details below to determine when, or if, you should expect a card for your various health plans. If you have questions about your insurance card or accessing care while waiting for a card, submit a submit a Synopsys Assist ticket.

If your address changes, you must update your information in Employee Central as soon as possible to ensure that you receive important benefits communications.

UnitedHealthcare (UHC) Medical

If you enroll yourself or a dependent in a UHC medical plan for the first time, you’ll receive an ID card in your name (your covered dependents, if any, will be listed on the card under your name). You’ll receive one card for employee-only coverage and two cards for employee + dependents coverage. If you’re already enrolled in a UHC medical plan and change to another UHC medical plan, you’ll have an updated digital ID on the UHC website and app. You may request that a new ID card be mailed to you if you prefer a physical card.

If you enrolled in a UHC plan for the first time during Open Enrollment, you’ll receive ID cards in the mail — one card for employee-only coverage and two cards for employee + dependents coverage. Expect your new physical ID cards and digital ID cards by the end of December. Upon receipt, register on the UHC website, and download the UHC app to access and manage your benefits.

To view your digital ID card or request a physical ID card, log in to UnitedHealthcare. You can also access your ID card digitally at any time on your UHC mobile app. Allow at least seven business days from the date you submit your enrollment for processing before you try to access your digital ID card if you enrolled midyear.

Kaiser Medical

You’ll receive an ID card only if you enroll yourself or a dependent for the first time. The plan issues new members an ID card in each member’s name (employee and dependents), since each member has a unique ID number. You can access your digital ID card on the Kaiser mobile app. If you enrolled midyear, allow 10 business days from the date you submit your enrollment before you try to access your digital ID card.

If you enrolled in Kaiser for the first time during Open Enrollment, you’ll receive ID card(s) in the mail by the end of December. Upon receipt, register on the Kaiser website, and download the Kaiser app to access and manage your benefits. 

Delta Dental

You’ll receive an ID card only if you enroll yourself for the first time. The plan issues new members two ID cards for all coverage levels. They’re issued in your name only.

To print your card, visit Delta Dental. You must first register at the site to access a printable card.

VSP Vision

VSP does not issue cards. You’ll just need to provide your full name, date of birth, and last four digits of your SSN so the vision provider can find your benefits coverage in the VSP system.

Fidelity Health Savings Account (HSA)

A welcome kit from Fidelity, containing a debit card, will be sent to you after your Health Savings Account (HSA) has been activated. You can request up to four additional cards by visiting Fidelity or calling (800) 544-3716.

HealthEquity Health Care Flexible Spending Account (FSA)

If you enroll for the first time, you’ll receive one card. You can order additional cards or replacement cards through HealthEquity. If you were enrolled in a Health Care FSA in the current year, you’ll continue to use the card you already have.
